The form is related to work within the Crescent concept of watershed management.

Only complete this form for streams in forested areas likely to be included in Ancient Forest Analysis. Complete one form per Stream.

1. Is there large woody debris (the result of logging, storms, or catastrophic fire) present in the stream? Y or N? (item 10 in program results)

2. Is the stream velocity reduced by more than about 10 % by the debris (if any)? Y or N ?(item 9)

3. Is the stream more than four (4) times wider than it is deep? Y or N ?(item 2)

4. Are there many pools in each stream "reach" (each mapped stream segment)? Y or No? (item 5)

5. Are there small pool areas? Y or N ?(item 5)

6. Are the pools that are present relatively deep? Y or N (item 3)

7. Are there scour pools? Y or N ? (item 9)

8. Are there diverse pool types> Y or N? (item 6)

9. Are there small riffle areas? Y or N? (item 7)

10. Are large proportions (greater than 10%) of the stream bed covered by fine sediments? Y or N (item 8)

11. Seining done? Y or N

If Yes to question 11, then answer: Are there many fish species (grest fish richness)? Y or N (item 4)
